About Us: The All Age Disability Service at Southwark offers a seamless and integrated social care pathway for children and young people with Learning Disabilities and other disabilities. We are committed to delivering a person-centered approach, promoting independence, positive outcomes, and the use of assistive technology.
The Role: As a Qualified Social Worker, you will be responsible for providing comprehensive and effective integrated social work service to meet the needs of children and young people with disabilities. Your role will involve undertaking strength-based social care assessments, developing outcome-focused support plans, and delivering focused interventions to safeguard and empower service users.
Responsibilities:
- Take responsive action in accordance with statutory provisions and departmental policies to ensure individual needs are met.
- Work within a systemic framework to deliver an effective integrated social work service in a timely manner.
- Make decisions on assessing individual needs and providing services, ensuring a strength-based approach to assessment and support planning.
- Develop outcome-focused support plans, empowering individuals to be active and engaged within their local community and maximize their independence.
- Engage directly with service users and their families, conducting accurate strength-based assessments and gathering information to capture their voices.
- Utilize various professional skills to achieve positive outcomes for vulnerable children, young people, adults, and families/carers.
- Devise, implement, and review outcome-focused care plans, including the management of protection plans within a child protection framework.
- Manage a caseload of children and adults, organizing and prioritizing work activities within appropriate statutory timeframes.
- Ensure timely delivery of statutory responsibilities, including visiting, arranging reviews, reporting to the court, and meeting performance targets.
- Provide reflective case discussions and group work when appropriate to support practitioners and service users.
- Establish and support local groups, customer representatives, and resources providing informal networks of care.
- Maintain up-to-date case records and management information on appropriate systems.
Requirements:
- Social Work qualification and registration with Social Work England (SWE) - ESSENTIAL
- Demonstrated experience in social work, particularly with individuals with disabilities.
- Sound understanding of relevant legislation, safeguarding policies, and statutory guidelines.
- Ability to conduct strength-based assessments and develop outcome-focused support plans.
- Background in Early help/ Safeguarding ideal
