Discharge and Flow Team - Empowering Independence
Reablement is a short-term and therapist-led service working with a remit to break free from ongoing services by promoting independence! We are a team equipped with providing information, needs analysis, equipment, adaptations and targeted support.
Since March 2020, we have teamed up with Health to transform hospital discharges in Wiltshire. The Wiltshire Locality Hub orchestrates discharges seven days a week from acute hospitals and facilities beyond county borders.
As a Discharge and Flow Manager, your mission will be to ensure seamless service delivery and craft innovative solutions for complex discharges of adults not fitting the residence criteria. You'll be the champion of integrated collaboration with community health teams and acute hospitals that interface with Wiltshire, navigating high-pressure situations with professional judgment to maintain hospital flow while ensuring customer safety during the transition home.
We're on the lookout for someone with a relevant professional qualification in social care or health, HCPC registration, and a Level 5 management qualification or its equivalent. If you've got at least 2 years of management under your belt and can showcase your skill in social care, reablement, legislation, and government policy, we would love to hear from you!

This position comes with a 12.5% market supplement payment. All market supplement payments are subject to review on an annual basis.
