The post holder will support their peers and be an effective team member working reporting to the Service Manager. Working on their own initiative and supporting the delivery of the triage function.
They will have a commitment to collaborative working and actively supporting and liaising with other health and social professionals and agencies.
Main Aspects of role
· The triage process is online based and will involve speaking to young people, parents/carers, school staff and other professionals in order to ascertain need and link young people into appropriate support
· The triage process is intended to quickly build a picture of the wishes, strengths and Mental Health needs of the children and young people who are referred to CYP MHTNS so that a prompt and positive outcome to the referral can be reached.
· Many of the children, young people and families who are referred to CYP MHTNS require light touch signposting and advice to help them thrive. Others require a Mental Health intervention from (CAMHS), Early Intervention Service or another partner organisation. The triage clinician will help to determine which of these outcomes is the most suitable for each young person.
Triage Clinician Role and Responsibilities
· To prioritising own day to day workload in the allocation.
· Check new online self-referrals for risk to ensure urgent needs are identified in a timely manner.
· Gather accurate and relevant information to enable timely and positive referral outcomes.
· Communicate referral decisions in a clear, accessible and helpful manner in writing.
· Respond to requests for support (calls/e-mails/letters) from parents/carers and professionals.
· Contribute to triage meetings/referral discussions and other forums/meetings.
· Keep clear records of information gathered, next steps, and decisions made.
· Build a strong understanding of available services in the LLR area.
· Develop a good knowledge base of on-line resources, apps, and websites.
· Record patient related information onto an electronic patient management system (System one) in a timely and accurate fashion in line with information governance protocols.
Other responsibilities:
· Build and maintain effective working relationships with partners and stakeholders.
· Submit timely and accurate monitoring data as required.
· Collect feedback and qualitative evaluations from young people, family members, staff and professionals and engage in service review and development processes.
· Manage time pressures effectively, and meet objectives in the face of competing priorities, challenging timescales, and diversions.
Leadership
· Support the development of an effective team and the development of productive
· working relationships.
· Actively promote integrated health professional working internally and externally.
· Participate in the audit process, linking in with the clinical governance agenda.
· Advise, encourage, and share clinical knowledge with peers and junior staff.
