Part time (Approximately 24 hours per week over 3 days, to be confirmed at interview) The position will require you to work 2 days per week at Fromeside with the remaining hours at Vinney Green Secure Children's Home. (Working hours and days can be agreed following successful interview but must fall within Monday to Friday 08:00-16:00) Up to £34,000.00 per annum (pro rata) Registered General Nurse or Allied Health Professional (depending on experience), Up to £24,000 per annum (pro rata) Nursing Associate (depending on experience). Key Duties and Responsibilities Clinical: Within the scope of your clinical competence and training, you will: Maintain your professional registration in line with requirements of the relevant professional body (e.g., NMC, HCPC) Participate in the safe and effective delivery of clinical care services, including but not limited to: Long-term condition reviews General health checks and NHS health checks Screening services Immunisation and vaccination Planned and acute wound care Health promotion and well-being initiatives Triaging patients and signposting / referring as per local pathways Assessment and treatment of acute injury and/or illness Responding to medical emergencies Basic life support Undertake comprehensive assessments of patient needs, including capacity and risk assessment Support patients to manage their own health and promote self-care Assist the social care provider with some social care activities, where required and appropriate General: In accordance with relevant policies, protocols, procedures, guidelines and standards, you will work under the direction of senior staff and in collaboration with the wider team to: Constantly strive to improve services delivered by Hanham Health Develop, implement, and review policies, protocols and standard operating procedures, within own scope of knowledge and competency Promote and support multidisciplinary and interagency working across the service Work proactively and collaborate with other agencies Promote and support effective links with relevant health and well-being services in the community, in support of continuity of care Understand the contribution of others in the wider secure setting system Contribute to strategies designed to promote and improve health and to prevent disease Adhere to national and local key performance indicators Identify issues pertinent to the patient population and advocate on behalf of patients to promote health and well-being Participate in patient feedback processes to support service improvement Participate in clinical audits to support service improvement and to comply with national and local requirements Ensure that all services and care is delivered in an effective and timely manner, in accordance with the organisations contractual obligations and with patient needs Participate in supervision and continual performance development reviews to ensure high quality nursing care Debrief and provide emotional support to colleagues following incidents and emergencies Positively support and inspire colleagues to improve standards and quality and to develop professional practice Recognise and take appropriate action on ethical and legal issues which have implications for clinical practice Identify and take appropriate action to protect vulnerable individuals Identify and take appropriate action on self-harm and suicide risk Report risks and incidents in support of a transparent, blame-free, and shared learning culture React to and report any threat, or indication of threat, to the security/safety of the establishment or any group or individual Comply with hospital orders, procedure, instructions, and security requirements Attend multidisciplinary team meetings and care planning reviews to provide input and advice on physical health concerns Treat all patients as individuals, always respecting their privacy and dignity Understand and adhere to all organisational policy, protocol, and procedure at all times Undertake such other duties as appropriate to the needs of the organisation In light of the changing needs in secure healthcare, this job description could be subject to change in the future.
