Supported by qualified social/health care professionals, you'll assess the needs of adults in the community and provide them with support and guidance to maximise their wellbeing. Using a person-centred approach, you'll use your knowledge of strengths-based practice to focus on people's resourcefulness and abilities and enable them to develop the best strategies to face life's challenges and live independently and safely in the community. You'll also need strong relationship management and negotiation skills to collaborate with service users, their families, providers and other health care professionals. As a Senior Case Worker you'll support more complex cases and provide coaching and mentoring support to Case Workers.

As part of your role, you will also take part in the duty rota for the Intake Team working alongside the Duty Manager. Whilst on duty, you will have the responsibility of effectively prioritising and responding to requests for support from individuals being referred to the team. Our Intake Team undertake telephone triage, well-being checks, Care Act Assessments under the Care Act 2014 and referrals to internal and external agencies. This team has fixed workers that undertake this role on a daily basis and the community team support with this when required.
