Job description
As a Case Worker in one of our Physical Disabilities Community teams, you'll use a strengths-based approach to assess an individual's needs and provide them with support and guidance to maximise their wellbeing. Strengths-based practice focuses on people's resourcefulness, strengths and abilities to enable them to develop the best strategies to face life's challenges and live independently and safely in the community. Supported by experienced colleagues and qualified social/health care professionals, you'll use your effective relationship management and negotiation skills to collaborate with individuals, their families, providers and other health care professionals.

Our experienced Case Workers can be considered for our social work apprenticeship scheme, which is currently run by the University of Winchester leading to a career as a Qualified Social Worker. Travel Requirements
You will be required to travel to various locations within Hampshire or other local authority, and it is essential that you have a current drivers' licence and access to a reliable vehicle on a regular basis.
