We are a well-established integrated service of enthusiastic Occupational Therapists, Physiotherapists, district nurses, matrons, case managers and health and well being workers. The South Therapy Team and neighbourhood holistically assess and treat people at home where it is safe to do so to reduce unnecessary hospital admissions and occasionally facilitate early safe discharge. You will have experience of working collaboratively within a community or acute setting, have excellent assessment, intervention, and communication skills. You will be able to carry out comprehensive assessments and make autonomous decisions based on the data obtained. We will provide interdisciplinary induction, clinical supervision, and opportunity for clinical skills training. This is an exciting opportunity to further develop your clinical assessment and negotiation skills. The South Neighbourhood Team provides an alternative pathway that will treat people at home where it is safe to do so to reduce unnecessary hospital admissions and facilitate early safe discharge where appropriate. Supporting people to remain out of hospital and maintain their independence is challenging but immensely rewarding and we would welcome informal enquiries and discussion prior to applications being submitted.
