The people we support often require wrap-around holistic support and have complex needs. Our Support Workers will carry this out through a variety of means, including co-ordinating support plans, carrying out home visits to our residents, encouraging engagement in Handcrafted activities and supporting personal development. First and foremost, we work with people who experience severe isolation, which means establishing a personal connection will be an essential foundation to overcoming the many and various barriers they face.
Responsibilities
Supporting & Collaborating
- Welcome and support people to integrate into the Handcrafted community and access the opportunities and support offered by Handcrafted
- Work with hub leads to identify opportunities to provide housing to people and oversee the support that is provided as they start and maintain sustainable tenancies in safe and comfortable housing environments
- To ensure the well-being of residents in our supported accommodation, to develop and regularly review safety and inclusion assessments and support plans that are tailored to the unique needs of each individual
- Meet weekly to plan support activities, and make any required changes to the support plan, personal details, or needs assessment
- Conduct regular checks of our properties to identify potential safety and security concerns
- Help people to discover the strengths and abilities that they have and how they can use these to help themselves
- Come alongside people during complex crises to provide moral and practical support, assessing and managing the risks associated with safeguarding issues
Developing & Partnering
- Promote Handcrafted's services, and those of other appropriate support organisations, to people who are socially isolated or vulnerable
- Cultivate partnerships with other organisations, donors, and supporters, communicating clearly how we contribute to further their aims
- Maintain records of contacts and plans for all referrals to ensure information is accurate, timely and relevant
- Undertake training courses and additional experience, tailored to your personal development, provided through Handcrafted and our partner organisations
General
- Attend team meetings as required to engage with other staff and volunteers, and to pray about Handcrafted, its residents and trainees
- Promote the vision and values of Handcrafted through all aspects of your work
- Any other duties as reasonably requested by the Hub Manager
