The Dementia Reablement team provide short term (up to 12 weeks) support to individuals recently diagnosed with dementia. The team provides information, advice and practical support to enable individuals referred to the service to continue to live independently for as long as possible and to continue to enjoy a good quality of life. The team provides specialist, short term interventions that are designed to equip people with early dementia, their carers and their families, with the practical skills, tools and information that can improve outcomes for the person affected.

An exciting opportunity has become available to work within the Care4CE Dementia Reablement service. The position is for a permanent Grade 6 Senior Support Worker, 25 hours per week, primarily working Monday to Friday 9am – 5pm, although some early evenings may be required. The service is based in Crewe Lifestyle Centre, but the team are Agile Workers so can also work from home at times.
