Your Role
The ARCH team work in close partnership with the local hospitals to support safe and timely discharges for people under the discharge to assess model. Your role will be to undertake assessments and reviews in line with The Care Act 2014, support people to access services and work to their strengths and assets, signposting to other services and/or providing information and advice when applicable.
About You
The ideal candidate will have transferrable skills of working in a person-centred role, this could be via paid employment or life experience. You will need to have good communication skills, be open to learning opportunities, and be able to work flexibly, alongside working well as part of a team and also as an individual.

We are a supportive team; we work closely with our partners from health and third party providers such as Age UK, housing, and care call. In doing so we are able to provide wrap around care and support. We can draw from others expertise to build our own knowledge and understanding whilst sharing our knowledge to inform others. We are based across several sites in Stockport, from hospital bases to residential care settings, and home and office bases. We are excited to be working at a time of review and change in service process and provision, ensuring we are providing the best service we can.
