How you'll make a difference
You will make a real difference with vulnerable adults in our community by assisting them in their time of need providing support and promoting their independence.
What will you be doing?
- You will assist in the organising and providing of effect care services which may include the assessment and review of care plans
- Working under direction of Social Worker/Occupational Therapist you will contribute to and review plans and services to improve intervention ideas.
- You’ll refer issues if needed in accordance with Safeguarding procedures to protect vulnerable people. This will require you to maintain confidential reports to profession and Council guidelines.
- You’ll be communicating, providing advice and guidance to with service users and providers
- Daily, you’ll be liaising and working with partner agencies, such as Health to ensure best outcomes for people.
What we need from you
- You will have experience in a relevant area of statuary or voluntary sector work in health and Social Care.
- You will demonstrate knowledge of relevant legislation, regulations and codes of practice; processes and procedures; support mechanisms and specialist equipment.
- You will need excellent interpersonal skills.
- You will be able to gather information from a variety of sources and follow assessment process
- Ideally, you will have relevant qualification e.g. NVQ Level 3 in health or Social Care field
Please note that due to the nature of this role it is a requirement of employment that an enhanced Disclosure and Barring Service (DBS) check is obtained for this post.
