Job description

How you'll make a difference

You will make a difference as a Social Care Practitioner in our Council with passionate service delivery, working alongside a highly supportive and committed team, who work with people over the age of 18 in supporting them to live at home by promoting their independence.  You will focus on ensuring that you contribute a positive approach in supporting and meeting the needs of our residents in the community.   
                                               

What will you be doing?

The Hospital Social Care Team consists of Social Work Practitioners, Occupational Therapists, and Social Care Practitioners. We use the 3 Conversation model to work with people following discharge from hospital, completing conversations and assessments in multiple settings including care homes and rehabilitation units across the whole geographical area. 
 

  • You will deliver genuine people-centred, strengths-based support, with a focus on what those we work with tell us will make the biggest difference to them, using your creativity to deliver the best outcomes for local people. 
  • You will assist in the organising and providing of effect care services which may include the assessment and review of care plans
  • Working under direction of Social Workers, you will contribute to and review plans and services to improve intervention ideas. 
  • You will be working closely with acute and community health partners, supported by our friendly team who work at pace.     
  • You’ll refer issues if needed in accordance with Safeguarding procedures to protect vulnerable people. This will require you to maintain confidential reports to profession and Council guidelines. 
  • You’ll be communicating, providing advice and guidance to with service users and providers  
  • Daily you’ll be liaising and working social workers and external contacts such as education services and health agencies ensuring communication is effective.                  

What we need from you

  • You will have experience in a relevant area of statuary or voluntary sector work in health and Social Care
  • You will demonstrate knowledge of relevant legislation, regulations and codes of practice; processes and procedures; support mechanisms and specialist equipment.
  • You will need excellent interpersonal skills.
  • You will be able to gather information from a variety of sources and follow assessment process 
  • Ideally, you will have  relevant qualification e.g. NVQ Level 3 in health or Social Care field 

Please note that due to the nature of this role it is a requirement of employment that an enhanced Disclosure and Barring Service (DBS) check is obtained for this post.
 

You will be expected to travel throughout the authority and surrounding area so must have a full valid driving licence with regular access to a vehicle.or have alternative appropriate means of travel.
