Social Work Assistant - Child Disability and Transition team

Maternity cover up to 12 months
22.5 hours per week
Our Child Disability and Transition team is a frontline assessment, case management team which specialises in providing support and services to disabled children and young people across the County Borough of Bridgend. This includes children who are care experienced and those disabled young people who are transitioning into adulthood.
The team responds to children and families with complex needs or where there are early child protection concerns and provides high quality support and intervention for those with longer term needs.
The team supports disabled children and young adults and their families through the process of transition into adulthood promoting independence where possible and identifying and accessing support services where required.
This is an excellent opportunity to join our busy, motivated and supportive Child Disability and Transition team as a Social Work Assistant. You will have the required qualification, experience and skills together with knowledge and understanding of the complex needs of disabled children/young people/adults and their families.
You will manage a caseload of Care and Support cases and also provide support to Social Workers in the team with more complex cases. You will undertake direct work with children and young people to achieve identified outcomes in their care plan.
This post offers a rare opportunity to work with both disabled children and adults and to gain a wide range of case management experience across the age span within a supportive team.
