We have a fantastic opportunity available for a Social Worker to join our Integrated Adult Mental Health team at Wokingham Borough Council, on a full-time, permanent basis.
In your role as a Social Worker, you willprovide specialist Social Care to people who have serious or enduring mental health conditions, working closely with health colleagues, and focusing on statutory duties under the Care Act, MCA, and MHA.
You will join a passionate and dynamic team, playing an integral role as we collaborate with The Health Trust to transform our services and develop a dedicated Social Care pathway.

Key Responsibilities:
- To hold a mental health caseload of varied complexity, providing care and services for people to assist them in their recovery.
- To adopt a strengths-based approach to the role and maintain a professional and respectful working relationship with individual customers and carers.
- To work collaboratively with individuals, their carers, families and other stakeholders to gather information for the purpose of assessing and reviewing the individual's needs and to identify the best options available.
- To effectively manage time, priorities, workload and conflicting pressures and escalate if appropriate.
- To establish professional relationships with relevant stakeholders including voluntary services to enable collaborative multiagency working.
- To proactively engage in relevant meetings and 1:1's and contribute to the development of the service.
- To produce care plans and ensure their effective implementation.
Candidate Requirements:
- Registered with Social Work England, and have experience working in a health or Social Care setting.
- Knowledge of Adult and Children Safeguarding Processes and procedures.
- Be a skilled communicator, with the ability to work collaboratively with colleagues and customers.
- Be well organized with excellent time management skills.
- Strong IT skills.
