Job Role: Experienced Social Worker(adults-5 years of experience)
We are looking for an Experienced Social Worker to work within a Busy Hospital Team within the Adult Social Care Team at Bracknell Forest. You will need to have good knowledge of Hospital Discharges D2A experience, Ability to complete Mental Capacity Assessments, Safeguarding experience. Must have a driving licence and car as working will be between Office and based at the Hospital. Must be able to apply a strength-based approach, using the conversations model to connect people to their community/social network, and problem solve together.
As a Social worker you must have a degree in Social work with a minimum 2 years post qualifying experience and passed your AYSE, experience in an NHS Establishment or a local authority is desirable.
To act in accordance with SWE registration requirements and Code of Conduct for exercising accountability and professional practice.
To provide professional Social work assessments to support the commissioning process in line with care management and continuing care.
Employers will be keen to see that you have a friendly and caring attitude, and understand and respect people who need healthcare support.
Previous experience and/or qualifications in Social work are essential to apply for this role.
