The Role
This is a permanent Social Work role in Adult Social Care. The post holders will be based in the Intensive Support Team, a division of Bolton Community Learning Disability Service.
You will work with the Intensive Support Team which specialises in supporting people with a diagnosis of Learning Disability who present significant behavioural challenges to services.
This will include those who are within, or at risk of being in contact with the Criminal Justice System, secure services and/or admission to a hospital under the Mental Health Act 1983.
Your role will ensure compliance of statutory duties in accordance with the Care Act 2014, Mental Capacity Act 2005; Mental Health Act 1983, NHE Transforming Care Agenda and relevant legalisation and practice guidance for this particular costumer group. i.e. criminal justice legislation.
You will work within an MDT to provide effective , timely hospital discharges or placement planning, using the relevent legislitive framework for placement both in and out of borough.
You will work closely with commissioning, gathering intelligence and working to develop/design effective systems of support for this particular cohort to ensure effective commissioning and develop market shaping.
You will be responsible to coordinate and providing key updates for customers on the Transforming Care tracker including as necessary reporting to the ICB, and the Greater Manchester Health and Social Care Partnership.
You will also be responsible for the governance of Mental Capacity compliance namely the Court of Protection application’s/ COP DOL renewals.

About You
We are looking to recruit enthusiastic and motivated individuals who have a passion for working with adults with Learning Disabilities, understanding their needs, aspirations and supporting them to enhance their quality of life.
You will have the ability to develop and maintain relationships with team members, external organisations, service users and carers. You will have the skills to be actively involved in multidisciplinary team working and decision making.
You will be able to remain focused and motivated, manage and prioritise your workload, in a hybrid working environment encompassing both office and non-office-based working.
Experience of resettlement from out of area placements back into the local area, understanding of legal frameworks around Capacity and Court of Protection, is preferable but not exclusive and, we welcome applications from newly qualified Social Workers and offer an extensive package of support through ASYE and for Continuing Professional Development.
