Pay Rate: £40 per hour (assignment rate)

Contract: Locum, 3-Months

Main Duties:

• To respond appropriately to referrals allocated by the Team Manager/Senior Practitioner.
• To plan, attend, chair or contribute to discharge planning and multi-disciplinary meetings.
• To ensure all assessments undertaken maintain independence, autonomy and dignity of the individual and draw upon the individual’s informal network where possible.
• To independently manage, plan and prioritise a caseload which will include complex casework.
• To discharge from the caseload those service users who are not currently active; this may involve transferring cases within the agreed timescales for further support internally or to other agencies as appropriate.
• To attend and contribute to weekly team meetings and group supervisions, and to partake in the team’s duty rota.
• To record factual information electronically, providing accessible assessment and care plan documentation to clients to meet their needs.
• To support each service user through their journey, ensuring the service user and family, if appropriate, understand the plans in place and have a copy of assessments and care plans if they so wish.
• To make ongoing referrals to services to achieve maximised independence e.g. Adult Learning Disability Services, Autism Initiatives.
• To have an understanding of mental capacity best practice and best interest decision making and to adhere to the Departmental Capacity Guidance.
• To be committed to work in partnership with other agencies and professionals to ensure best positive outcomes.
• To challenge poor/unsafe practice within care provision and report this to management.
• To undertake Adult Safeguarding referrals as required.

Requirements of the Social Worker:

• Degree or equivalent in social work
• Current Social Work England registration
• Significant frontline experience in Adults Services within the UK
