Job Overview:
Working in a busy adult social care team to ensure new referrals are assessed, advice and support provided and annual reviews completed. Safeguarding and Duty work be required.
Experience of using LiquidLogic (LAS) recording system essential.
Duties:
- Conduct assessments to identify clients' needs and develop individualized care plans
- Provide counseling and emotional support to clients
- Connect clients with community resources, such as housing assistance, healthcare services, and financial aid programs
- Advocate for clients' rights and help them navigate complex systems
- Collaborate with other professionals, such as healthcare providers and educators, to ensure comprehensive care for clients
- Monitor clients' progress and make adjustments to care plans as needed
- Maintain accurate and up-to-date client records
Experience:
- Bachelor's or Master's degree in Social Work or related field
- Previous experience working with children, families, or individuals in need is preferred
- Knowledge of social service programs and resources
- Strong communication and interpersonal skills
- Ability to work effectively in a team environment
- Empathy and compassion for others
Job Types: Full-time, Temporary contract
Contract length: 3 months
Pay: £27.00-£33.00 per hour
Expected hours: 37 per week
