We are currently looking for a dedicated Social Worker to join our Older People's Mental Health team on an initial 1 year fixed term contract.
As a Social Worker within the OPMH Team, you will be working with our residents that have either Dementia, Alzheimer's or another Mental Health diagnosis. You will be ensuring that any issues are dealt with in a timely manner and their families and carers are supported throughout any admissions or intervention.
As part of your job role, you will be working alongside a variety of professionals from different organisations to help in developing a service that provides the best possible Social Care Services for our service users.
The successful candidate will be responsible for carrying out community care assessments, designing and implementing support plans for the residents in their care.
You will be ensuring that Thurrock residents with Mental Health illnesses are supported to live in their own homes for as long as possible, you will ensure that these residents are not admitted inappropriately or prematurely to hospitals or care homes and those that are admitted are discharged in a safe and timely manner.

- Social Work Degree
- Social Work England Registration
