An exciting opportunity has arisen for a Social Work Practitioner in the Older Persons Team, South Powys, covering Hay and Talgarth , Brecon , Crickhowell, Ystradgynlais locality.
The Older Persons Team work with people over the age of 65 years and their carers, using a Collaborative & Strengths -based, Outcome focused model of practice.
About you:
- You will have excellent communication skills and have experience of the social care sector.
- You will have knowledge and understanding of the Social Services and Well Being (Wales) Act 2014 and 2005 Mental Capacity Act 2005.
- You will be able to work collaboratively to achieve best outcomes for people.
What you will do:
The post holder will be undertaking holistic and strengths-based assessments and reviews. The post will entail working with individuals to help them find appropriate solutions at times of personal challenge, as well as in the long term. It will include identifying and raising concerns where individuals may be experiencing harm or neglect.
The post holder will be expected to build effective working relationships with individuals, their carers and other professionals.
