About us
We are a dynamic, welcoming team working with adults with Learning Disabilities, their carers and families. We support people from age 18 years onwards and work with children’s services supporting young people with Learning Disabilities.
The team is made up of Team Manager, Advanced Practitioners, Social Workers, Social Care Facilitators and an Occupational Therapist. We undertake assessments, support planning and reviews under the Care Act 2014, working closely with Health partners, Education, SEN, Schools and Colleges, Children’s Services, Transition and Leaving Care Workers, Commissioners, and other external agencies to provide an integrated approach to supporting individuals. Working in a strengths-based and risk enabling manner to achieve best outcomes and increase independence.
About you

We are looking for enthusiastic and committed professionals to join our team, to help shape and deliver our service, and contribute to the development of a model of best practice. The successful applicant will be skilled in carrying out strengths-based assessments and support planning under the Care Act 2014, completing comprehensive Risk Assessments, Mental Capacity Assessments and Best Interest Decision Making. You should have experience of working within a multi-disciplinary setting, to include health partners, voluntary and community organisations.
We are looking for forward thinking team members who will promote and deliver positive outcomes for individuals and their carers, we are currently planning changes to support this by having named workers to ensure greater consistency. Therefore, you will need to demonstrate experience of working with people who have Learning Disabilities. Integral to this role is having a comprehensive working knowledge and understanding of Making Safeguarding Personal and experience of undertaking safeguarding enquiries.
