You will be part of the Social Care Assessment Service consisting of senior practitioners, Social Care practitioners and other Social Workers who are dedicated to providing outstanding care within the community, working Monday-Friday 9am-5pm (some flexibility offered) with no weekend or bank holiday working.
Your role
We are a short-term service providing support to service users after discharge from hospital, working closely with the reablement service as part of the discharge to assess model.
As a Social Worker in the Social Care Assessment Service, you will:
Hold a case load of clients, carry out Care Act Assessments
Be responsible for Care Planning and case management of adults including being lead worker for safeguarding enquiries
You will be based primarily in Peasedown St John, Bath but will work across the whole of Bath and North East Somerset region.
About You
You must be a qualified Social Worker and be registered with Social Work England.
Ideally you will have knowledge in social work assessment and discharge within a hospital team however, if you do not have this experience yet, please still apply as we are committed to providing training and development to further your potential and support your progression.
