Your main responsibilities as a Social Worker:
- To assess and care manage the complex needs of Older People and their carers to ensure their health, wellbeing and social inclusion.
- To liaise with Reablement and other professionals to ensure seamless care pathways which safeguard and promote independence.
- To promote partnership working which supports the person centered outcomes to be delivered for Older People.
- To undertake safeguarding investigations as required.
What we are looking for in a Social Worker:
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK licence
- Post-qualified experience
- Degree level or equivalent in Social Work
