Job description


Are you a Band 5 Occupational Therapist looking to develop your skills at a Band 6 level? Are you a Band 6 Occupational Therapist who would like to develop your clinical skills in a different area?

If so, we have a permanent full time position available here in the District Therapy Team in Bolton.
We are seeking to appoint a dynamic, confident, innovative and flexible individual with excellent communication and organisational skills. The successful individual will need to engage closely with acute staff, community colleagues and social care staff to coordinate and provide high quality care in the community to avoid hospital admission and support hospital discharges.

You will need some knowledge of community/intermediate care services. You will be an effective team player, inspirational, flexible, adaptable, enthusiastic and fully committed to the development of patient focused, clinically effective and efficient services.
The role will be part of a multi-disciplinary team consisting of Physiotherapists, Occupational Therapists, Therapy Instructors, Nurses, Pharmacy and Local Authority staff. You will be a key player in providing assessment, personalised care planning and management of care to patients who may require intensive home support as an alternative to hospital care. The role will include participating in effective multi-disciplinary team meetings and close working with the Local Authority Reablement team.
You are encouraged to maintain your Covid vaccination status unless medically exempt and will be supported to access information and guidance, to inform decisions that supports vaccine confidence, protects yourself and patients.

The service is currently operating shifts over 7 days between 8am and 6pm and you will need to be a car driver with business insurance.
