Specialist Pharmacist
BAND:7 (£43,742-£50,056)
LOCATION:Base site LRI/LGH/GH as per needs of relevant Clinical Management Group (CMG), but expected
to work across the Trust as required
HOURS: Up to 37.5 hours per week (plus weekends, bank holidays and on call services as per departmental
rotas)
REPORTS TO:CMG Lead Pharmacist
ACCOUNTABLE TO:Chief Pharmacist
You will utilise your clinical experience by providing daily clinical pharmacy services to specified wards and associated areas, assisting with the training and development of others and supporting the improvement of the services.
You will work closely with the multidisciplinary teams and will help maintain the high standard of services that are already provided. There is an ever growing number of Independent Prescribers in our services and a strong desire to increase these numbers further so that responsibility for prescribing becomes integral to a pharmacist’s role.

Post holders will be expected to contribute towards evening, on-call and weekend rotas.
• To deliver a clinical service to patients, including taking patient drug histories, rationalising prescribing, advising on and compiling treatment protocols, therapeutic drug monitoring and monitoring compliance.
• To support the CMG Lead Pharmacist in maintaining and improving the pharmaceutical service to the clinical management group (CMG).
• To be responsible for the delivery of an agreed standard of pharmaceutical care to identified patients in order to achieve the safe and cost effective use of medicine.

Detailed job description and main responsibilities:
· To deliver a clinical service to patients, including taking patient drug histories, rationalising prescribing, advising on and compiling treatment protocols, therapeutic drug monitoring and monitoring compliance whilst in the relevant CMG rotation.
· To participate in the delivery of optimum pharmaceutical care to patients served by the clinical directorate on specified ward(s) / clinics and deliver a level of clinical input commensurate with the grade.
· To contribute to the management of the directorate drugs budget via regular expenditure analysis feedback and participate in structured ongoing drug use audits to ensure optimal drug usage.
· To provide education and training to all other healthcare staff as appropriate
· To participate in late duty, weekend and bank holiday cover in accordance with departmental rotas.
