Description
We are currently seeking a highly skilled and compassionate Specialist Social Worker to join our team. As a Specialist Social Worker - Adults MH & LD (Grade 9), you will have the opportunity to make a significant impact on the lives of adults with mental health and learning disabilities. You will work closely with individuals, their families, and other healthcare professionals to assess needs, develop care plans, and provide ongoing support.
In this role, you will play a crucial part in the delivery of person-centered care, ensuring that individuals have access to the support and resources they need to thrive. You will advocate for your clients, facilitating communication and collaboration between various organizations and services. Additionally, you will provide counseling and therapy, helping individuals to manage their mental health and improve their overall well-being.
Responsibilities
Assess and evaluate the needs of individuals with mental health and learning disabilities
Develop and implement care plans tailored to meet the individual's specific needs
Provide counseling and therapy to individuals and their families, utilizing evidence-based techniques
Advocate for individuals, ensuring that their voices are heard and their rights are upheld
Collaborate with healthcare professionals and other organizations to coordinate services and support
Monitor and review care plans regularly, making adjustments as necessary
Provide crisis intervention and support during times of acute mental health or behavioral challenges
Requirements
Bachelor's or master's degree in Social Work or a related field
Current state licensure or certification as a Social Worker
Demonstrated experience working with adults with mental health and learning disabilities
Strong knowledge of mental health conditions and treatment options
Excellent interpersonal and communication skills
Ability to work effectively as part of a multidisciplinary team
Sound judgment and decision-making abilities
Empathy and sensitivity towards individuals with mental health and learning disabilities
Hours Per Week:
Start Time: 08:30
End Time: 17:00
Pay Per hour: £27.98
Location: Hull, East Riding Of Yorkshire
