Provide written, as well as verbal specialist advice in relation to the care of patients on discharge home from the UTC/MIUs. To provide health promotion advice to patients and their families/carers to support the maintenance of their health and well-being. To act as a mentor and resource for more junior colleagues, supporting and assisting them to provide care of the highest quality.
