The post holder will use experiential knowledge and clinical skills to ensure effective application and interpretation of the National Framework for the NHS Continuing Health Care and NHS Funded Nursing Care. The post holder will work closely with hospitals, community services and care providers to develop systems and processes to facilitate a smooth assessment and referral process under the supervision of the Senior Nurses. The post holder will complete non-complex decision support tool meetings and formulate recommendations for panel consideration. The post holder will regularly review, under the supervision of the Senior Nurses, the clinical needs of the patient with regard to appropriateness of the placement, level of care being provided and entitlement to NHS Funded care or Continuing Healthcare. The post holder will require good verbal and written communication skills to communicate with patients, their families and other professionals. The production of accurate, detailed and comprehensive reports is essential. Complete CHC and FNC Nursing Needs assessments, reviews and checklists for patients in accordance with the National Framework for NHS Continuing Health Care and Funded Nursing Care. Complete assessments as required for appeals or retrospective work as applicable. To support the duty nurse function as required and undertake other duties at the request of the line manager/supervisor, which are commensurate with the role. Communicate and assist colleagues and stakeholders around issues within own area of work. Communicate and co-operate with internal and external staff, patients, carers, and families.
