Outreach Practitioner

Service Care Solutions are supporting a local Drug & Alcohol Charity and they are currently recruiting for a Outreach Practitioner. As a Outreach Practitioner you will play a crucial role by supporting individuals that are suffering with Drug & Alcohol addiction.
The Ideal candidate needs to be experienced within the field but also someone who is highly driven and can identify positivity within a service user and have the ability to develop potential within.

Key role and responsibilities for a Outreach Practitioner;

• Able to manage a small caseload of 14-15 clients


• Provide Outreach Support


• Build a support Network


• Deliver One to One interventions

Minimum requirements for a Outreach Practitioner;

• Experience of working within the Substance Misuse field


• Awareness of health issues relating to Substance Misuse


• Experience of Key Working and Care Planning
