Elizabeth House has a exciting opportunity for a Support Planner role working within the core service. We deliver housing, health, employment and support to people with a diverse range of needs. We empower people to achieve financial stability, social inclusion and independence.
Elizabeth House is a well-established service based in Daybrook providing accommodation and support for homeless people from the Broxtowe, Gedling and Rushcliffe areas often with a range of complex support needs. We are looking for reliable, enthusiastic and self-motivated candidates who can demonstrate an understanding of the issues facing those affected by homelessness and who want to work in a fast-paced environment. Excellent communication skills, resilience and a positive approach are essential.
Duties & Responsibilities
• To provide support to Service Users in a safe and maintained environment, by support planning, reviewing and assessing their individual risks and needs.
• Prepare Support plans.
• Signpost Tenant’s onto appropriate services for substance misuse management & health.
• Assist service users with housing applications and move on options.
• Assist service users with basic budgeting and life skills in order to support them to move on to live independently.
• To liaise appropriately with key partners, stakeholders and service providers to ensure appropriate support is offered to all Service Users.
