Support Worker

Are you an experienced Support Worker who would like to work for a values led organisation?

We have a fantastic opportunity for a Support Worker with good initiative and great communication skills to join our busy and friendly team in Fleetwood.

You will assist in the delivery of a stable and effective programme by the efficient delivery of the day-to-day service. You will also support Service Clients to achieve the aims laid out in their Care/Support Plan. Your professional and flexible nature will allow you to positively impact on those who you support and the rest of the team around you, allowing you to flourish and develop in this diverse and rewarding role.

We have a great opportunity available for a Support Worker based in Fleetwood on a permanent basis, working 40 hours per week.

KEY RESPONSIBILITIES

• Key working and supporting clients generally within the service and acting as the Key Worker to designated clients as and when required
• Taking an active role in the assessment and management of risk
• Regularly fully utilising the Trust’ Client File’ with designated clients to ensure they are fully supported and empowered
• Providing support and guidance to clients
• Ensuring that all medication within the service is managed in line with Trust policy and procedures
• Ensuring that clients are able to participate as fully as possible in the life of the project
• Managing client meetings and facilitating the Client Inclusion and Training representatives
• Providing other staff and clients with relevant advice, support and training
• Promoting and supporting client involvement, managing complaints and seeking resolutions
• Supervising and monitoring the behaviour of clients
• Assisting in the management of the referral process within the service
• Ensuring that both prospective and former residents are supported

Park View is a 10-bed 24/7 Supported Housing Service based in Fleetwood, with an additional 14 beds in the community. Clients are ex-offenders with complex needs and associated risks, and require an empathetic approach to support their continuing rehabilitation. At times, the ability of staff to deescalate tensions, and minimise client’s anxieties through providing an approachable, listening ear is part of ensuring the safe risk management of the service. This is also a part of helping clients to rebuild their confidence and their lives.
