We are a Domiciliary Care & Supported Living Provider looking for carers to support people with a wide range of additional needs. We are expanding and are providing Supported Living for adults 18+. Applicants will have a genuine interest in caring for adults and have previous experience and understanding of learning disabilities and Mental Health
Main Duties
- Supporting Individuals: You’ll work closely with the people you support, ensuring their well-being and satisfaction. This involves enabling them to make choices, promoting independence, and maintaining their health and safety.
- Personal Care: If required, you’ll provide personal care, which may include tasks like toileting and bathing, following the individual’s support plan.
- Home Management: You’ll assist individuals in managing their own homes, including supporting them with maintaining tenancies and providing guidance during home transitions.
- Health Support: This includes helping with health needs, such as administering medication and accompanying individuals to doctor’s appointments.
- Financial Assistance: When necessary, you’ll support individuals with their finances, following guidelines set by the organization.
- Promoting Independence: Encouraging participation in everyday activities, both at home and in the community, is a key aspect of the role.
- Collaboration: You’ll work positively with colleagues, families, and external professionals to provide holistic support.
- Confidentiality: Maintaining confidentiality is crucial in respecting individuals’ privacy.
- Continuous Learning: Participating in learning and development opportunities ensures you stay informed and improve your support skills
- Collaborating with Healthcare Professionals: You’ll work closely with other healthcare professionals to develop personalized treatment plans for individuals with Mental Health conditions.
- Implementing Treatment Plans: Administering medication or therapeutic care to patients based on the treatment plans.
- Observing Behavioural Changes: Monitoring patients for any behavioural changes and addressing them promptly.
- Supervising and Ensuring Safety: Supervising and monitoring patients within the facility to ensure their safety.
- Risk Assessments: Performing comprehensive risk assessments on patients.
- Educating Patients’ Families: Educating patients’ families about their loved ones’ Mental Health issues and providing guidance on how they can support the treatment process.
- Documenting and Maintaining Records: Keeping accurate records of patient health, progress, and any incidents.
- Referring to Specialists: Recommending patients who need more personalized treatment to Psychiatrists and Clinicians.
REQUIREMENTS
- Experience in a similar role is desirable but not essential.
- Compassion, patience, and a positive attitude.
- Excellent communication skills.
- Ability to work independently and as part of a team.
- Driving License and own car preferred.
