Job description

Key Duties: Clinical: 1.1 To act as a case manager for own caseload, with support from the Band 7 nurse specialist and reference to NICE Guideline 2016, co-ordinating and providing care with the community (including home visits and seeing patients in other agreed locations)# 1.2 Ensure accurate, legible and timely documentation relating to all aspects of patient care and treatment both paper and electronic. 1.3 To support patients in adhering to treatment regimens by assessing and developing strategies to overcome barriers to adherence. 1.4 Provide screening/immunisation clinics for TB contacts and new entrants as required, children and adults. 1.5 Work under patient specific/group directions to perform and assess Mantoux testing, interpreting results appropriately, administering BCG as required or sending for further investigation in accordance with NICE TB guidance (2016) 1.6 Maintain effective communications with the patients, families and all members of the multidisciplinary team to ensure that appropriate information is shared and the patients needs are meet 1.7 Support, organise and where appropriate, manage Directly Observed Therapy Short course (DOTS/VOTS) within the community for all sources of Pulmonary TB infection to prevent multi-drug resistant (MDR) strains of TB.
1.8 Carry out contract tracing (statutory responsibility) thus preventing the spread of the disease. 1.9 Respond to local policies on screening of patients identified as at risk. 1.10 Support, reassure and educate the infected person and family/contacts. 1.11 Develop and maintain a sound knowledge base and practise relevant to the TB service 1.12 Facilitates a multi-professional approach to the management of TB/NMTB/LTBI patients across Worcestershire.
1.13 Support and help implement changes in clinical practice based on current initiatives and research 1.14 By clarifying healthcare needs, to act as an advocate for the client group with the application of ethnic, legal and professional knowledge and skills. 1.15 To support useful links with non-statutory bodies to achieve better service delivery 2. Non-clinical 2.1 To co-ordinate and lead the TB service in the absence of the Band 7 Tuberculosis Specialist Nurse including supervision of other staff members.2.2 To be able to work independently under the guidance of Band 7 TB nurse specialist as well as within a team 2.3 Input data related to Tuberculosis notifications according to relevant protocols. 2.4 Competent in the use of the IT systems including: ICE, Oasis PAS, Bluespier and Microsoft Office.
2.5 Provide information in a variety of languages on Tuberculosis and Immunisation. 2.6 Liaise with the multidisciplinary team within the Tuberculosis Service, covering Worcestershire and UKHSA. 2.7 Provide statistical returns as required by the service. 2.8 Attend and participate in all clinic meetings and patient meetings as agreed with Manager.
2.9 Share clinical knowledge with colleagues and be involved in the delivery of ongoing training, education and support of TB awareness sessions and study days. 2.10 Participate in clinical audit and research as required, relating to the TB speciality. 2.11 To participate in developing, writing and maintaining team and local policies and protocols 3. Education 3.1 Ensure that best practice guidelines are adhered to and remain up to date with current tuberculosis screening, diagnosis, and treatment.
3.2 Prepare for and participate in annual appraisal process. 3.3 Mandatory training requirements will be undertaken; all other training will be identified as part of the annual appraisal process. 3.4 Attend regular training and update sessions as agreed with Manager. 3.5 To participate in training of new members of staff and students, acting as a mentor to students as required.
4. PROFESSIONAL 4.1 Maintain a high level of awareness of relevant research, issues, trends and national recommendations within the area of TB practice and in nursing. 4.2 Take an active part in professional networks to ensure the exchange of knowledge and ideas, both locally and nationally. Contribute to the development of national policy 4.3 Take responsibility for personal development and education regarding Revalidation for the NMC.
4.4 Participate in clinical supervision for self and others and utilise reflective practice techniques 4.5 Participate in National Strategies and initiatives, local and regional cohort review. 4.6 To act within the boundaries of the Code of Conduct and the Scope of Professional Practice as defined by the Nursing and Midwifery Council (NMC)
