Job summary
The Enhanced health in Care Homes (EHCH) framework is for primary care networks (PCNs) and providers and commissioners of community health and social care services.
Sussex Community Foundation Trust have an important part to play in ensuring this framework is successful and alongside our established Care Home Matrons (CHM) an important and exciting element of EHCH is to introduce telemetry opportunities into the Care Homes both residential and nursing homes. We've completed Phase 1 focusing on Nursing Homes and to commence Phase 2 which will focus primarily on Residential Care Homes.
Enhanced Health in Care Homes is looking to recruit a Band 6 Lead Nurse to take the Telehealth agenda forward and the role will entail driving across East and West Sussex to visit individual Care Homes.
Early deterioration of the health of residents, and subsequent early intervention by appropriate health provider could reduce unplanned conveyance to hospital.
The acceptance of Telemetry is voluntary in the Care Homes but those who've accepted may have varying levels of Digital experience so a calm and approachable manner is essential to the success of the initiative.
The role is at the forefront of Digitally enhanced healthcare and an exciting opportunity for an experienced Registered Nurse looking for a new experience.
Main duties of the job
The role is operational Monday to Friday, 37.5 hours/week excluding Bank Holidays.
The Lead Nurse (LN) will line manage a B4 Telemetry Co-ordinator, non clinical role , and a B3 Project Co-Ordinator.
Work with and support the Care Homes in the devising and implementation of contingency plans and pathways, with the emphasis on the "What Next?" viewpoint, to reduce unplanned hospital admissions.
Support change management within Care Homes to understand and try working in new ways, yet maintaining patient safety and well-being.
Manage their own workload and have oversight of the teams efficiency to ensure the ideals of the project are met.
Have a calm and approachable, yet professional manner with good negotiation skills in supporting Care Homes to work in a new way.
Signpost and support Care Homes to obtain the appropriate On-Line training provided by the Integrated Care Board (ICB) and provide reassurances and advice as indicated.
Training and Induction will be provided on commencement.

Job description
Job responsibilities
The Lead Nurse (LN) will manage a small team of a B4 Telemetry Co-ordinator, non clinical role who will facilitate the installation and functionality of the Telemetry system, and a B3 Project Co-Ordinator who will provide administrative duties, and be a first contact with the Care Homes early discussions for implementation and setting up appropriate appointments for the B4 and LN visits.
Line management of the team will include supervision, appraisal and ensuring the team remain up to date with mandatory and statutory training and education in line with their role. To manage sickness and annual leave effectively, and within SCFT's Policies.
Although not a budget holder will need to have oversight that Trust funds and assets are managed efficiently and within budget.
Ensure accurate and contemporaneous records and documentation are kept by all team members.
Implement a robust internal escalation system is in place and team members work within the remit of their roles.
Confidence to drive within the Sussex counties all year round. Extreme weather is addressed within SCFT policy.

  • 3 years post registration experience

  • Currently a Band 6
