Servoca are working with a national organisation who are looking for a Team Leader to join the team in Leyland who are supporting 12 young care leavers.
You will be responsible for supporting the young people who have been in local authority care as they prepare to move on from their services and make the transition into adulthood.
Preparing the young people to live independently by providing advice, information and practical support.
Companionship and emotional support, contribute to the young persons care planning, eventual and achievement of goals.
Identify, assess and respond to changes in the young persons physical, emotional and mental health needs.
Coordinate with other support workers and family members to ensure the young persons needs are being met.
Assisting with the completion of household tasks, errands and shopping.
