Address We have an exciting opportunity for a dynamic Rehabilitation Support to join our Admission Avoidance and Discharge Service (AADS) in East London NHS Foundation Trust. This role is based in the Mile End Hospital at borough of Tower Hamlets and is part of the Admission Avoidance & Discharge Service (AADS) which consists of a team of multidisciplinary professionals that work across the community and hospital to facilitate early discharge and prevent unnecessary hospital admission. The service uses the discharge to assess model and works closely with our social care partners, including the Reablement service and hospital social workers.
We are looking for a highly motivated, innovative, and flexible rehabilitation support worker to join our team in working with patients across a variety of clinical areas. As a successful candidate, you will work under the direct supervision of registered health care professionals in managing complex cases and facilitating rehabilitative programmes in patient’s home environment to work towards patient’s centred goals as set out in their support and treatment plan. The team has an excellent working relationship with hospital staff, GPs, the extended primary care teams, ambulance crews, the Physician Response Unit, care home staff and other community services.

The Admission Avoidance & Discharge Service (AADS) currently includes an Admission Avoidance Team based in the Emergency Department, In-reach nurses, Rapid Response and the Intermediate Care Team that uses a “discharge to assess model”. The main aim of the team is to reduce hospital stay and admissions by supporting clients in their own homes. The team includes a triage and screening Hub, which provides timely triage, assessment, onward referral and signposting for service users referred to appropriate community team without the need for a multiple referral.
The post holder will work as a part of the multi-disciplinary AADS team and work alongside our acute colleagues and other key stake holders to identify, assess, and coordinate seamless and safe discharge from hospital to residents of Tower Hamlets. The post holder will provide generic skilled support work to aid the AADS team and will be expected to undertake a number of duties without direct supervision but will be required to report back to a qualified therapist. This will include an initial assessment of patients within set parameters and implementation of rehabilitation programmes in order to facilitate clients reaching their optimum level of independence and social integration.
The post holder will be competent to work with occupational therapists, physiotherapists, nurses, and rehab support workers undertake various rehabilitation programmes and equipment ordering and provision.
The post holder will work independently as part of the AADS team carrying a delegated caseload, under the guidance and supervision of a qualified member of staff, within set parameters providing treatment regimens as monitored by qualified staff. The post holder with be responsible for time management, planning, and organization of their daily workloads as delegated by a qualified staff member.
The post holder will be an active participant in the late shift of 12-8pm and weekend rota including bank holiday.
The post holder will be responsible for reporting on patients’ progress and changes to function to qualified staff and relevant team members and professions.
The post holder is responsible for their continued professional development including participation in therapy training programmes.
