We are looking for a well organised, enthusiastic and motivated individual to join our team in Discharge Planning. This role will include a wide range of duties to assist in the day to day running of Discharge Planning.
We are looking for an administrator with a positive approach and excellent inter personal/communication and IT skills. The role is varied and you would provide administrative support to our Discharge Planning Team liaising with colleagues in a wide range of groups and organisations.
Main duties of the job
To support the management of a professional administration service to the Discharge Planning team, to all of its users including patients, relatives and colleagues from both internal and external organisations.
Working with support and minimal supervision, to undertake a variety of tasks to support patients discharge pathways.
Working within specified areas, to undertake a variety of delegated tasks to support the delivery of the service to patients, in accordance with agreed guidance/care pathways.

Educated to GCSE level or equivalent.

Experience

Experience of working in a busy office environment or reception role.
