Brook Street Ltd are pleased to announce a readily available Admin position at NHS in Fareham close to train station

This role offers a unique opportunity to contribute to the improvement of patient outcomes through efficient placement support and care procurement.

The role is based on a full-time hybrid long temporary on-going assignment, Monday-Friday, standard office hours. The pay rate is 10.42ph for the first 12 working weeks. Please note: once you have been in the assignment for 12 working weeks, your hourly rate will increase to 11.67/hr.

The Placement Support Team plays a vital role in facilitating the timely discharge of individuals from hospitals, particularly those on the Discharge to Assess pathway or with standalone health needs. Reporting to the CHC Commissioning Team, this position collaborates closely with the Central Assessments Team to manage daily referrals and resolve complex cases efficiently.

Key Responsibilities:

  1. Support care procurement for hospital discharges and standalone health needs, ensuring adherence to clinical guidance and commissioning appropriately.
  2. Manage NHS placement contracts, ensuring agreement within the pricing structure.
  3. Implement policies to minimize delayed hospital discharges, optimize block purchased capacity, and commission cost-effective care.
  4. Achieve Key Performance Indicators, such as timely referral-to-discharge duration and efficient use of block purchased capacity.
  5. Effectively communicate with stakeholders, including care providers, Commissioning Team, Central Assessments Team, Hospital Discharge Teams, Single Points of Access, and Local Authority.
  6. Identify best value options when spot purchasing at pre-agreed rates.
  7. Issue individual placement agreements to providers for NHS-funded placements and care packages.
  8. Compassionately engage with service users, families, and representatives in accordance with commissioning and choice policies.
  9. Ensure accurate and up-to-date data systems and client records for reporting purposes.
  10. Facilitate funding arrangements prior to placement and maintain confidentiality of patient information.
  11. Handle inbound and outbound telephone calls and emails independently, managing queries and stakeholder expectations.
  12. Prioritize workload, react quickly to urgent escalations, and adhere to challenging deadlines.
  13. Participate in team and 1:1 meetings with the Line Manager.
  14. Attend mandatory and role-relevant training.

Qualifications:

  • Strong interpersonal and communication skills.
  • Professional telephone and email etiquette.
  • Proficient in IT skills, including database updates, Excel spreadsheets, and Word documents.
  • Ability to work in a fast-paced environment and thrive in challenging situations.

Pay rate - 10.42ph. Please note: once you have been in the assignment for 12 working weeks, your hourly rate will increase to 11.67.

Hybrid role but training in the offices and may be expected to go in a couple days a week

Long temporary assignment - pay 1 week in arrears, vacancy will be until end of June, with potential extension pending probation period.

The position is subject to a basic DBS check and 5 year's referencing.
