Encompass delivers a wide range of services including Adult Social Care and homelessness support.
For this job role, you will be working in the Adult Social Care Brokerage Team who arrange care for customers in their own homes or in residential care settings at the request of Social Workers for Sutton Council.
In this job role you will:
- Work via a duty system including telephone enquiries, emails and direct referrals
- Be responsible for assessing client circumstances and potential barriers to support
- Find and facilitate access to accommodation & other support as appropriate
- Ensure person-centred arrangements match Support Plans
We are looking for someone who has:
- Knowledge of the 2014 Care Act
- Knowledge of the 2005 Mental Capacity Act and Deprivation of Liberty Safeguards
- Possible experience in the care sector
- A customer focused attitude
- Ability to build positive relationships
