About the role

We are looking for an Adult Support Coordinator to join us in our North Transfer of Care Team.

We are a busy team, covering Hinchingbrooke, Peterborough City and Queen Elizabeth (King's Lynn) Hospitals and two smaller rehabilitation wards.

Our role is to support adults being discharged from the acute hospitals and rehabilitation settings. Please note that the main base for this role will be Hinchingbrooke Hospital but there will be an expectation to cover the other  Hospitals on an 'as required' basis.

We do work weekends, but you are given a day back during the week and a salary enhancement too!  Our staff receive a salary enhancement between 7% and 15% depending on their selected working pattern.

  • 7% for 1 weekend day a month and 1 Bank Holiday a year
  • 8% for 1 weekend day a month and 2 Bank Holidays a year
  • 10% for 1 weekend day a month and 3 Bank Holidays a year
  • 15% for 2 weekend days a month and 3 Bank Holidays a year

The role involves completing Care Act Assessments, Mental Capacity Assessments, Safeguarding enquiries, care planning, working closely with the multidisciplinary team to identify appropriate pathways for discharge and ensuring unpaid carers are supported by having carers conversations. All in a time limited environment.  The role involves working with people to identify their strengths and how these can be used to support them in meeting their assessed needs. This is all in line with the Care Act 2014, as part of strengths-based and person-centred interventions.

The post holder will work with vulnerable adults with a range of social care requirements including; older adults, people with disabilities and mental health needs.

Closing date 27th March.  Interview will be held virtually via Microsoft Teams on 3rd and 4th April.  There will be a short written exercise as part of the interview but we’ll give you all of the relevant information ahead of time.

What will you be doing?

You will be responsible for completing holistic social care assessments under The Care Act 2014, and Mental Capacity Assessments to determine the level of Care and support needed for Cambridgeshire patients/service users being discharged from the acute hospitals, rehabilitation units and those that have been discharged on a health interim pathway.

You will be the lead professional supporting people and their families with all aspects relating to their care and care planning to support timely discharge from Hospital.

 

About you

You will need to understand Adult Social Care legislation including the Care Act 2014, Safeguarding Adults and Making Safeguarding Personal principles as well as the Mental Capacity Act 2005.

You will be expected to work collaboratively as part of a team and demonstrate commitment to making a difference and building relationships. 

You must have the ability to travel to a variety of locations independently.

You must be able to commit to working in the office 5 days a week during the Probation period so as a team, we can support your learning/development.
