Job Role:  Adults Social Worker (Locality team)
HOURS: full time mon-fri 9-5
To promote and maintain independence for vulnerable adults using a strengths-based approach. 2. To protect adults at risk by the use of procedures to safeguard adults from abuse. 3. To assess the needs of individuals and their carers in line with Care Act (2014) criteria 4. Together with the service users and carers, formulate and implement person-centred care and support plans 5. Monitor, review and evaluate services in order to ensure that they are provided in accordance with eligible needs.

As a Social worker you must have a degree in Social work with a minimum 2 years post qualifying experience and passed your AYSE, experience in an NHS Establishment or a local authority is desirable.
To act in accordance with SWE registration requirements and Code of Conduct for exercising accountability and professional practice.
To provide professional Social work assessments to support the commissioning process in line with care management and continuing care.

Employers will be keen to see that you have a friendly and caring attitude, and understand and respect people who need healthcare support.

Previous experience and/or qualifications in Social work are essential to apply for this role.
