2 x Social Worker - Telford Integrated Community Assessment Team (TICAT)

37 hours per week (weekend working) and fewer hours/job share can be considered)

Qualified PO2 / Experienced PO3/ Advanced Practitioners PO5
NJC Grade PO2 (£36,648 to £39,186) per annum plus Market Factor Payment (£1900).
NJC Grade PO3 (£37,336 to £40,221) per annum plus Market Factor Payment (£4505).
NJC Grade PO5 (£39,186 to £42,403) per annum plus Market Factor Payment (£4505).
Salary pro-rata for less than 37 hours per week.

Do you have a passion for supporting people through strength-based practice to be as independent as they can? Being in hospital can be a very difficult time for individuals and their families. Social Workers are integral to getting people back home with a good network of support around them. Telford Integrated Community Assessment Team (TICAT) supports all our pathways from hospital. You will be part of a multi-agency team, using strengths and asset-based approaches; focusing on how individuals can be supported by the strengths they have in themselves, their network of support from family, friends, and their local communities.

Where care and support are required, we work to ensure that this reflects the individual’s assessed needs and strengths while promoting their independence, choice and control. We encourage positive risk taking with a ‘home is best’ philosophy, this is reflected in our performance as Telford and Wrekin is proud to have a lower than national average per population of admissions into long term placements for older people.
