Job description
The post holder will work as part of the multi-disciplinary intermediate care services to utilize specialist physiotherapy clinical knowledge and skills for a range of clients to improve the quality of their lives, increase their ability to live as independently as possible and minimize longer term dependence on Health and Social Care Services. The post holder will be required to work with a range of clients receiving Intermediate Care Services, and must ensure that their practice is based on current research evidence, clinical audit and professional best practice. The post holder will develop and maintain effective communication with all relevant stake holders in Primary and Secondary care, other Statuary Organizations, the Community and Independent Sectors as appropriate. The post holder will use a client centered approach focusing on individual client needs and goals. The post holder will work autonomously at a high clinical level and are expected to act as a source of clinical expertise to provide an advisory, education and consultative service to members of the team, less experienced physiotherapists/undergraduate students and other professions. The post holder will demonstrate a high level of clinical reasoning/diagnostic skill holding an active caseload of clients mainly with highly complex needs and offering comprehensive holistic assessment, treatment and management skills.

A minimum of 5 years post graduate clinical experience with at least 3 years at senior level (band 6 level or above) of which at least 12 months is in the treatment and management of patients within acute/elderly medicine/surgery providing rehabiliSuccessful completion of a MSc module relevant to the clinical area e.g falls, frailty, rehabilitation or a willingness to undertake within 2 years of commencing the post , Have worked in a physiotherapy role within a community setting
