Key details/responsibilities:
The purpose of this role is to provide community-based alcohol interventions to people who have been assessed in hospital by the Alcohol Care Team (ACT) following alcohol related hospital admissions. Postholders will carry out assessments and interventions predominantly in people’s own homes as well as other community-based settings.
This role will work closely with the hospital ACT to ensure appropriate procedures are in place for community discharge, and handover of ongoing treatment needs/ recovery.
