Assistant Practitioner : £29,269 to £31,364 per annum (Scale G)

An exciting opportunity has arisen within the Adult Mental Health Social Care service to be part of recently developed Centralised duty team for Mental Health. The successful applicant would be joining the Central Duty Team which currently covers North Norfolk, South Norfolk and Norwich localities ( there is a plan to expand this to cover East and West Norfolk )

You would be part of a small and developing team of practitioners who respond to referrals received into the mental health team and you would be required to adopt a strength based approach as outlined in the Care Act 2014. You would be required to have good risk assessment skills and be able to work under pressure.
The successful applicant would be required to work collaboratively with colleagues in a fast paced environment, liaising with a number of professionals from both within Norfolk County Council and external agencies. Whilst the majority of the work will be over the telephone, there are occasions where face to face assessments and interventions are required so the successful applicant would need to be able to drive and have access to a transport.

We are proud of our multi-disciplinary working environment and positive working relationships with our Health colleagues. Our team values good listening skills, flexibility, adaptability to new ways of working and a commitment to supporting people to maintain their independence. Workers in our team are open to change, innovative and confident with a focus on strengthening social care across Norfolk. Any new staff are supported to develop a thorough working knowledge of Norfolk County Council processes and procedures. We are investing in people who can take us forward on our journey to embed our vision. As an Assistant Practitioner within this team, you will undertake assessments, service commissioning and reassessment work. These will be carried out via video/telephone and/or face to face. You will have excellent communication, record-keeping and keyboard skills, along with experience of working with vulnerable people. In addition, you should have a commitment to excellent customer service and be familiar with the Departments' eligibility criteria. In return, we can offer you the opportunity to work in an exciting and challenging area of Social Care, where you will be supported to develop into the role with regular supervision, training and peer support.

All our office-based roles are currently working from home as part of our Smarter Working plans and to support the health and safety of all our colleagues. We anticipate that working remotely will be a key feature of this role. Our technology platform and equipment are first class enabling you to connect and collaborate remotely. We ask that you have in place good Broadband connectivity.
