Job Role: ASW - Social Worker LD Community Teamin Haringey
Our Social Work Team makes up part of a wider multi-disciplinary Learning Disability Partnership where we work alongside colleagues from local health trusts to ensure the holistic needs of our service users are met.
Within the Social Work Team there are multiple functions, and this role would be part of the ‘Transition’ element of the team. This involves supporting young people and their families to navigate the move between children’s services and adult social care.
You will bring the principles of professionalism, ambition, accountability, dignity, respect, empathy, and a strength based, collaborative approach to service delivery.
As a Social worker you must have a degree in Social work with a minimum 2 years post qualifying experience and passed your AYSE, experience in an NHS Establishment or a local authority is desirable.
To act in accordance with SWE registration requirements and Code of Conduct for exercising accountability and professional practice.
To provide professional Social work assessments to support the commissioning process in line with care management and continuing care.
Employers will be keen to see that you have a friendly and caring attitude, and understand and respect people who need healthcare support.
Previous experience and/or qualifications in Social work are essential to apply for this role.
