Locum Band 6Community Physiotherapist - Rapid Response, Falls Response, and Intermediate Care Service

Working alongside Rapid Response and intermediate care services in delivering innovative services including Stop & Drop with patients in their own home, Falls Response service and intermediate care/ Rapid Response caseloads.

Job Description:

As a Locum Band 6Community Physiotherapist within a Rapid Response Service, your role is to provide specialized physiotherapy services to individuals requiring falls response, Rapid Response, and intermediate care within the community setting. Working on a temporary or contract basis, you will fill in for absent or vacant positions, supporting patients in managing falls risk, providing timely interventions, and facilitating their recovery and functional independence.

Responsibilities:

  1. Falls Risk Assessment: Conduct comprehensive falls risk assessments for individuals referred to the Rapid Response Service. Identify factors contributing to falls and develop intervention plans to minimize falls risk and enhance safety.

  2. Rapid Response Intervention: Respond promptly to urgent referrals and provide immediate physiotherapy assessment and intervention for individuals following falls, accidents, or acute functional deterioration. Implement measures to support recovery and prevent further functional decline.

  3. Intermediate Care Planning: Collaborate with patients, their families, and the multidisciplinary team to develop individualized intermediate care plans. Provide physiotherapy interventions to support individuals in regaining mobility and facilitating their transition back to their home environment after hospitalization or acute events.

  4. Mobility and Gait Training: Provide targeted mobility and gait training to individuals to improve their walking ability, balance, and functional mobility. Utilize walking aids and assistive devices as needed to enhance independence.

  5. Activities of Daily Living (ADL) Training: Provide training and support to individuals in improving their independence in activities of daily living, such as dressing, grooming, and bathing. Implement exercises and strategies to promote functional abilities.

  6. Home Assessment and Modifications: Conduct home assessments to identify potential hazards and barriers to independence. Recommend environmental modifications and assistive devices to support safe and independent living at home.

  7. Rehabilitation Planning: Develop and implement rehabilitation plans tailored to the individual's needs and goals. Collaborate with occupational therapists, nurses, and other healthcare professionals to ensure a comprehensive approach to care.

  8. Family and Caregiver Education: Educate family members and caregivers on ways to support the individual's recovery and promote functional independence. Provide training in safe handling techniques and strategies for falls prevention.

  9. Pain Management: Use various physiotherapy modalities and techniques to manage pain and discomfort associated with falls and acute functional deterioration.

  10. Documentation and Record-Keeping: Maintain accurate and up-to-date documentation of assessments, treatment plans, progress notes, and outcomes measurement in compliance with legal and ethical standards. Ensure confidentiality and privacy of patient information.

  11. Continuous Professional Development: Stay updated with the latest research, advancements, and best practices in falls response, Rapid Response, and intermediate care. Engage in professional development activities, such as attending relevant training, conferences, and workshops. Seek supervision and engage in reflective practice to ensure professional growth.

Qualifications and Requirements:

  • Bachelor's or Master's degree in Physiotherapy.
  • Registration with the relevant professional regulatory body.
  • Car driver and car access essential in seeing patients in their own home.
  • Significant experience working as a Physiotherapist in community settings, preferably in falls response, Rapid Response, or intermediate care.
  • Knowledge of falls risk assessment, falls prevention strategies, and evidence-based physiotherapy interventions.
  • Strong clinical assessment and intervention skills.
  • Experience in working with individuals with acute functional deterioration and complex care needs.
  • Excellent communication, interpersonal, and teamwork skills.
  • Ability to work effectively within a multidisciplinary team and collaborate with other healthcare professionals.
  • Flexibility and adaptability to work within a locum or temporary role.
  • Adherence to professional ethics, confidentiality, and data protection regulations.
