SalaryFull details of the job responsibilities are contained within the available job description document and include: Obtain information from a wide variety of sources, exploiting all appropriate systems and resources, in order to meet organisational requirements Ensure data is collected in an accurate, timely, consistent way and that complete information is recorded within PCD systems Interrogate databases, collate and analyse data for monthly submissions and contractual KPI requirements, for both internal and external audiences Provide operational and performance data for quality and performance reporting, giving recommendations Conduct analysis at a strategic level to support the Senior Leadership Team, using analytical tools and techniques to interpret gaps, patterns and trends, and make recommendations in support of decision-making, prioritisation and resource allocation Communicate and work with project leads and service managers to identify data requirements, address any data queries and support decision making Identify data quality errors, take action to correct and escalate any data quality issues to the Senior Leadership Team as required Work with service and other managers and leads to develop operational and quality reporting structures and templates Work with the Contracts Manager to support with monitoring and reporting on KPIs within the contracts Work with South Yorkshire ICB to understand the variation of general practice service delivery across the City using the data produced by the SY ICB and other partner organisations Support the preparation of Primary Care Network level population health profiles (including a mixture of health and social measures), along with system partners, ensuring national products and horizon scanning form part of this preparation Lead on discussions with the Primary Care Network to compare data and identify where improvements can be made for the patient population Engage with Primary Care Network Clinical Directors and leads for iterative developments to the population health profiles
