Care Coordinator

You will join a forward-thinking ambitious team of Care Coordinators focussed on ensuring that the utilisation of nursing and care resources is both maximised and aligned with client requirements within defined quality, compliance, and safety standards.

What will you be doing?

The Care Coordinator will be responsible for the overall management and scheduling of client rotas to ensure that the care delivered is of the highest standard. You will work with all internal teams to facilitate patient centred care that enriches the quality of our client's lives and enables them to live within their own homes whatever their abilities.

  • Ability to multitask and work on different tasks simultaneously and to deadlines
  • Experience working in a healthcare setting is desirable but not essential.

Responsibilities

  • Point of contact for clients/families and all staff
  • Building relationships with all clients, families, and staff to create and maintain trust in all aspects of their packages.
  • Rostering all shifts for the packages they support and Responsible for covering missed shifts/annual leave & sickness with bank staff and or Agency support.
  • Working in partnership with the Clinical Leads to maintain the ongoing safety of the service users we support. Form interviewing, to group meetings and appraisals.
  • Completing supervisions for all staff to track their happiness and well being.
  • Responsible for capturing any client and carer feedback forms to help develop the service we offer to all.
  • Attending multi-disciplinary meetings with other professionals, including commissioners and medical professionals that help support the service users we look after on a day to day basis.
