The Client:
Our client, a reputable national franchise in the care sector, assists adults needing support to maintain independence in their private residences.
The Role:
As a Care Coordinator, you will work closely with clients, families, and carers deliver care and supervise care assistants for effective team performance.
Responsibilities:
- Update and revise clients' care plans and risk assessments.
- Monthly documentation and analysis of care outcomes.
- Consult with clients for care plan recommendations.
- Perform monthly checks on care equipment as outlined in care plans.
- Maintain precise records and reports.
- Fulfil on-call duties as per the assigned roster.
- Hold monthly meetings with the care manager.
- Adhere to established policies, procedures, and practices.
Requirements:
- Previously worked as a Care Coordinator or in a similar role.
- At least 1 year of caring experience.
- NVQ Level 3 in health and social care or working towards.
- Strong computer and administrative skills.
- Valid driving license and own vehicle.
- Right to work in the UK.
